We are curious how you experienced our care and services. That is why we ask you to complete a survey after the contact (by telephone, your visit to us or the visit of the doctor). Your opinion counts and gives us important information that we can learn from.

The questionnaire takes about 10 minutes to complete and is anonymous. This means that the data you enter cannot be traced back to a person.
